How do you get custom item frames/maps on the creative server?

You'd have to make a map and fill every block in that area with the color you want. Isn't perfect and is very time consuming, however.

I made probably 15 custom maps for item frames in Admins World. Probably half of the ones going around creative were made by me a year ago xD
